The Social Care Community Partnership (TSCCP) provides complex care for both adults and children with long term needs. As a registered provider we can deliver this service through local authorities, NHS trusts and private individuals.
TSCCP supply and manage specially trained social care staff who all work together with the service user, families and stakeholders to provide a person centred care service with choice and dignity at the heart of it. Each service user is an individual with specific requirements and each package of care is tailored to meet their needs.
